Анотація

Фібриляція передсердь (ФП) та когнітивні розлади (КР) на сьогоднішній день є поширеною проблемою, особливо серед осіб похилого віку. Численні дослідження останніх років демонструють взаємозв’язок між ФП та КP, зокрема з деменцією. З огляду на збільшення поширеності ФП з віком і постаріння населення прогнозують, що кількість осіб з КР зростатиме.

До основних патогенетичних чинників, що відіграють роль у розвитку КР при ФП, належать вогнищеві ураження (макро‑ та мікроінфаркти, мікрокрововиливи), гіпоперфузія головного мозку та системна запальна реакція. Безсимптомні (приховані, або «німі») інфаркти головного мозку, що є випадковою знахідкою під час проведення нейровізуалізації, трапляються частіше, ніж маніфестні церебральні інсульти, й асоціюються з КР. У пацієнтів з ФП їх виявляють частіше, ніж в осіб без ФП, тому, на думку багатьох дослідників, ФП є вагомим чинником їхнього виникнення.

Деменції, КР та старінню головного мозку можна запобігти. Для збереження здоров’я мозку найбільше значення мають здоровий спосіб життя (особливо здорове харчування, достатня фізична активність та нормальна маса тіла) і правильне лікування захворювань, які належать до судинних чинників ризику (артеріальна гіпертензія, цукровий діабет, гіперхолестеринемія та ФП). Є дані, що вчасно призначена антикоагулянтна терапія у пацієнтів з ФП може знизити ризик розвитку КР. Останні дослідження демонструють перевагу прямих пероральних антикоагулянтів над варфарином щодо профілактики розвитку КР у пацієнтів з ФП. Перспективними напрямами досліджень є вивчення нових (зокрема генетичних) біомаркерів високого ризику КР, порівняння прямих пероральних антикоагулянтів для визначення оптимальної профілактики КР, підвищення прихильності пацієнтів до терапії, застосування нефармакологічних стратегій для підтримки синусового ритму.
