Нейрорадіологічні ознаки енцефалопатії у дітей з розладами спектрa аутизму, асоційованими з генетичним дефіцитом фолатного циклу

Анотація

Результати 5 метааналізів вказують на асоціацію розладів спектрa аутизму (РАС) з генетичним дефіцитом фолатного циклу (ГДФЦ) у дітей. У таких випадках формується специфічна енцефалопатія з переважанням імунозалежних шляхів патогенезу, радіологічні ознаки якої недостатньо вивчено.

Мета — описати типові нейровізуалізаційні ознаки енцефалопатії у дітей з ГДФЦ і РАС та встановити кореляції між клінічними ознаками, механізмами пошкодження нервової системи і даними нейровізуалізації для оптимізації алгоритму діагностики, моніторингу та лікування.

Матеріали і методи. Ретроспективно проаналізовано медичні дані 225 дітей (183 хлопчики і 42 дівчинки) віком від 2 до 9 років з ГДФЦ, у яких відзначено клінічні вияви РАС. Діагноз РАС установлено дитячими психіатрами за критеріями DSM‑IV‑TR (Diagnostic and Statistical Manual of mental disorders) та ICD‑10 (The International Statistical Classification of Diseases and Related Health Problems). Патогенні поліморфні варіанти генів фолатного циклу визначали методом полімеразної ланцюгової реакції з рестрикцією. Нейровізуалізацію проводили за допомогою магнітно‑резонансної томографії головного мозку у конвенційних режимах (Т1‑ і Т2‑зважені, FLAIR) на томографах з величиною магнітної індукції котушки 1,5 Тл. Для вивчення асоціацій між досліджуваними показниками застосовували показник відношення шансів і 95 % довірчий інтервал.

Результати. Виявлено 5 основних груп нейровізуалізаційних ознак, характерних для лейкоенцефалопатії, скроневого медіанного склерозу, PANS/PITANDS/PANDAS, перенесених природженої СMV‑нейроінфекції та постнатальних енцефалітів, малих аномалій розвитку ЦНС. Нейровізуалізаційні ознаки тісно асоційовані з результатами спеціальних лабораторних тестів, які характеризують відомі імунозалежні механізми пошкодження ЦНС, і появою відповідних клінічних синдромів церебральної дисфункції, що узгоджується із сучасними концепціями основних інфекційно зумовлених або автоімунних уражень нервової системи у пацієнтів з імуносупресією. Виділено лабораторно‑радіологічно‑клінічні комплекси (вірусіндукований скроневий медіанний склероз, автоімунний лімбічний енцефаліт, автоімунний субкортикальний енцефаліт, автоімунне або вірусіндуковане демієлінізувальне ураження півкуль великого мозку, наслідки перенесених нейроінфекцій та малі аномалії розвитку головного мозку).

Висновки. Енцефалопатія у дітей з РАС, асоційованими з ГДФЦ, має складний патогенез і є наслідком різних комбінацій низки  імунозалежних форм ураження ЦНС, зумовлюючи гетерогенний клініко‑радіологічний фенотип.
